The Cancer Support Programme (CaSP) provides one-to-one support for people living with or beyond cancer to explore their needs and identify community services to help them mee their goals.
A service for cancer patients newly diagnosed or undergoing treatment (aged 18+) living in Staffordshire and attending Royal Stoke, County or Cannock Hospitals. 
The CaSP provides health and social care staff the opportunity to refer cancer patients for personalised one-to-one support in their community to overcome practical issues and improve their well-being through social and emotional support.
 The CaSP service supports patients to:
Ø    Self-assess their concerns and feelings (physical, practical, family/relationship, emotional, spiritual/religious, lifestyle or information needs)
Ø    Explore and define what is important to them 
Ø    Identify the local activities and resources they need
Ø    Gain support to engage with these services. For a referral form please email admin@bjf.org.uk or telephone 01782 844036. Referrals are welcome from professionals, family, carer and self.
(Permission must be obtained to give information to us if the referrer is other than either patient or carer)
